Position Summary: The Community Health Coordinator is responsible to support community-based public health services for Pic Mobert community members. This includes:

  • Delivery of health promotion, safety, disease prevention and health education programming at the community level;
  • Advocacy for clients with complex health challenges; assisting in the identification of health needs and supporting effective and efficient access for these clients to appropriate health and social services;
  • Development and delivery of timely public health campaigns that address critical issues on reserve;
  • Coordination support for programming and services for elders and disabled.

The CHC is responsible to be working in collaboration with the Dilico Nursing and PWS programs as appropriate. The CHC is responsible to gather information on stats, data, and services related to community and regional health, and linking with regional providers.


The Employee must meet the following minimum criteria:

  • Post-secondary education related to nursing, public health, health planning, education or an acceptable combination of education and experience;
  • Group facilitation skills relating to providing public programming at the school, within the community, and to families;
  • Proficient in use of computer programs related to writing, planning, and budgeting (Word, Outlook, PowerPoint, Excel, etc.);
  • Good written and oral communication skills as well as good interpersonal skills;
  • Proven ability to effectively prioritize work flow;
  • Ability to work as a productive member of the health team;
  • Ability to network with local and regional resources;
  • Ability to exercise good judgment, show initiative and be proactive;
  • High standards of ethics and confidentiality related to sensitive information;
  • Must have a reliable vehicle and valid class G driver’s license as some travel is required;
  • Must be in a state of personal health and wellbeing that will permit the individual to balance personal and work challenges and effectively manage the demands and stresses associated with working in this field within this community;
  • Must be able to pass pre-employment drug screening and obtain CPIC and Vulnerable Sector Check;
  • Must be available to work irregular hours in order to best meet client needs, including evenings and weekends.

Job Duties:

  • Health Canada Mandatory Health Program: assist the Community Health Nurse in all aspect of the Mandatory Health Program delivery as appropriate;
  • Provide health promotion, disease prevention and health education programming (school, individuals, group and family) to Pic Mobert First Nation members (i.e., dental health, injury prevention, life style counselling, etc.);
  • Ensure access to services for health clients in Mobert, including support related to transportation to/from services (working with Referral Clerk), general follow up and advocacy for care of people with chronic illnesses, care of the elderly, people with special needs, etc.;
  • Coordinate the Homemaking Program and case management for clients with chronic illnesses, care of the elderly, people with special needs, etc. (may involve access of special services, tracking, general follow up, advocacy and problem solving);
  • Develop and provide/ensure provision of community based programming and services related to diabetes in conjunction with the Dilico nurses;
  • Support Disease Prevention Programming in conjunction with the Dilico nurses: assisting with well child examinations, examination of school children, vision and hearing screening, examination of adults (blood glucose, blood pressure,) facilitate other screening programs as needed (i.e. head lice screening at the school), etc.;
  • Environmental Health: assist at the community level when required by liaising with the environmental health program (water, sewage disposal, garbage/solid waste disposal, housing and building inspections, etc., food quality control, pest control, animal control, etc.);
  • Take a lead role in the Elders programming as it relates to programming coming out of the Health Centre, such as the Elders Council, special projects or trips, etc.;
  • Build linkages with local and regional health and social services to maintain current statistics related to health and social issues/needs in Pic Mobert and in the region;
  • Maintain listings of current service providers and ensure linkages with health and social opportunities for the community;
  • Work with the Health and Social Manager to coordinate crises response and emergency response related to acute illnesses/conditions/situations: arrange for emergency transportation, assist with community emergencies such as fire evacuation, floods, storms, chemical spills, etc.;
  • As part of the public health oversight, as needed work with issues related environmental health (water, sewage disposal, solid waste disposal, housing and building inspections, food quality control, pest control, animal control, etc.);
  • Take a lead role in health careers promotion, work with funders to access funding for this event;
  • Coordinate the production of a monthly calendar of health programs and events;
  • Ensure appropriate, timely promotion of activities (Website, Facebook);
  • Maintain professional client records, documentation of concerns, activity records, monthly planning calendars, program reports as required;
  • Abide by the principles and behavioural expectations of client and program confidentiality;
  • Support Health and Social Program staff and activities as part of the staff team as required by the Health and Social Manager;
  • Be a role model in personal and family health through personal conduct within the workplace and outside of work in the community;
  • Other duties as directed by Health and Social Manager and Band Manager.
